All photos must have GPS coordinates embedded. The authorized verifier must complete the Employment Verification Form. They need to provide their full name, designation, department, official contact number, and official email address. They must also sign the form and apply the official clinic stamp or seal, which is mandatory. If any information is unavailable, the verifier must explicitly write "Records Not Available" rather than leaving fields blank.
